Optimize H1N1 Vaccine Process

Fairfax, VA.—December 3, 2009— Formatta, a leading provider of eForms-driven business process

announced today it was launching along with Document Conversion Associates, Inc. (DCA) of Lebanon, NJ,
an eForms solution to help speed the availability of H1N1 flu vaccine for County governments.

Using the Formatta solution implemented by DCA, County Health Departments across the United States will
be able to automate existing H1N1 Vaccine Consent Forms using the Formatta solution, allowing the
County to report vaccine usage to State and Federal health authorities in near real-time, allowing the County
to receive new shipments of the H1N1 flu vaccine more quickly.

Under the process optimized by DCA using the Formatta solution, an existing paper-based H1N1 vaccine
form is automated and is available electronically to be completed from any web browser. Once the form is
completed, the data is routed electronically to a database located on the county server; the form is printed
out, signed and brought to the clinic location. After the vaccine is administered, additional information such
as vaccine batch number, date and location of the injection on the body is noted. These handwritten notes
are then scanned and appended to the data collected electronically from the online form. This combined
data is then transmitted to the appropriate State and Federal agencies for reporting. A unique bar code
generated upon completion of each online form is the mechanism for matching the data collected
electronically with the handwritten information added to the form at the clinic location.
